How and what Personal data is collected

We collect your personal data when you choose to share it with us. By sharing your data you enable us to enroll you for policy, deliver our services and to allow us to communicate with you. If you are eligible for our services through your employer, your employer may provide us your following personal and sensitive personal data directly or via third party. In case you apply for individual plan, we may collect below information directly from you.

• Contact information (name, address, email address, contact number)
• General information (such as gender, age, date of birth, marital status, date of marriage)
• CNIC number, Bform number or Passsport number
• Applicable documents such as CNIC, Bform, passport, marriage certificate or birth certificate
• Medical and claim History
• Bank account details
• Employment information (such as employee ID, date of joining)
• Dependents/Spouse/Family details

We also collect information pertaining to your medical treatment for which your claim is received by us. This information includes applicable records such as medical history, diagnosis, reports, bills and receipts.

When you interact with EFU Health online services we may ask you for some of the above data. For example when you create account in EFU Myhealth app or portal we may ask your CNIC number, email address and mobile number. Furthermore we may also ask for your location when you access some of the features of EFU Myhealth app. When you request for addition of your family member, we may ask for some of the above information related to your family member.

Furthermore we also collect other information when you access EFU Health online services. This information includes your IP, browser info, device info, Page or feature you accessed and timestamp of your visit. In case of visiting our website we also learn about the referral used to reach our website.

To whom does EFU Health Insurance disclose information

The personal information we receive is considered confidential. We are committed to keep it secure and we do not share, sell or disclose it to any third party except:

• If required by government and regulatory body to meet applicable legal or regulatory obligations.

How we will use your personal data

By sharing your data you give us unrestricted rights to store, use, and process the information for following purposes:

• To register you as our customer/user
• To provide our services to you
• To provide you information related to your Policy, Medical plan and your claims
• To show you nearby network hospitals and discount centers
• To share with you our individual plans with premium
• To distribute/share any guidance or marketing material with you
• To inform you about our events, services, products and share newsletter with you
• To facilitate compliance with government and regulatory laws

How to request for deletion of personal data

You can ask for deletion of your personal data under certain circumstances. If you want to remove your account in our mobile app or portal, you can request us by sending an email to Customer.Relations@efuhealth.com. Your request will be processed within 15 days and we will send you confirmation once deletion is processed. Upon deletion of your account you will not be able to login in mobile app and portals but you will be able to signup again until your policy is active. Please note that in case you request deletion, we might keep some of the data necessary to keep our services available to you and for legal or regulatory obligations.

Your personal data is processed within the territory of Pakistan.

All content of EFU Health online services is protected by copyright with all rights reserved. You may not copy, modify, reproduce, republish, upload, post, transmit, or distribute any of its content including, but not limited, to Company Name, Logos, text and graphic images nor should you use links or reference in your materials, website or application without prior written consent of EFU Health. Some content on EFU Health website may be protected by copyrights of other companies.

Prohibited Conduct

By accessing or using EFU Health online services you are certifying that you will not be involved in any of the below conduct:

• Trying to gain unauthorized access to the restricted content and information
• Impersonate and use someone else identity to gain access
• Sharing of your access credentials with other person or entity
• Submitting misleading information or content to us through our services
• Altering, exploiting or interfering with services which harm the availability, confidentiality and integrity of the information and service

Term and Termination

Portal and some of the exclusive features in mobile applications will be available to our active policy holders only. Features may also vary for different users depending on their policies and coverage. EFU Health reserve its right to revoke access from portal and mobile applications in case your policy gets inactive or any violation of Terms of Use is noticed.

No Warranty

All content of the EFU online services, including, but not limited to, graphics, text and hyperlinks or references to other sites, is subject to alteration without prior notice and is provided on an “as is” basis and without warranty of any kind, expressed or implied.

We do not warrant the adequacy, accuracy, reliability or completeness of any information provided and expressly disclaim any liability for errors or omissions therein. We do not warrant that the services will be uninterrupted and/or error-free.

In no event, including (without limitation) negligence, will EFU Health be liable for any loss or damage of any kind, including (without limitation) any direct, special indirect or consequential damages, even if expressly advised of the possibility of such damages, arising out of or in connection with the access of, use of, performance of, browsing in or linking to other sites from the EFU Health Website.

Certain hyperlinks or referenced website on EFU Health website may take you to third party websites. The content of these websites is not maintained or analyzed by us, and we do not warrant the adequacy, accuracy, reliability or completeness of any information on hyperlinked or referenced websites and disclaim any liability for any and all of their content.
